Reporting Lost or Stolen Cards
If your card is lost or stolen:
- Contact U.S. Bank Customer Service Office at (800) 344-5696
- Report stolen cards to the local law enforcement agency
- Contact your account manager and the Purchasing Card Administrator indicating how the card or account number was compromised. Be sure to include the last 4 digits of the compromised card number in the e-mail.
Note: The UCD Police should be contacted if a Purchasing Card is stolen on campus, or used fraudulently on campus. In those cases, the cardholder should file a report with the police as soon as possible, as well as complete the other steps outlined above.
